What is Support Coordination?
Support Coordination involves working with participants to identify, coordinate, and access supports and services that meet their specific needs, wants, and preferences.
Your Support Coordinator will:
✔ Help you understand your NDIS plan
✔ Discuss the services you want
✔ Find at least three providers that can deliver the services you need
✔ Complete the necessary paperwork for your chosen providers
NDIS Support Coordination Levels
There are three levels of support coordination that you can include in your plan:
Level 1: Support Connection
This level helps you connect with informal community support and services to reach your NDIS goals independently. We’ll work with you to create your goals, and you can choose your preferred providers and coordinate your own support.
Level 2: Support Coordination
Support Coordination helps you understand your NDIS plan, navigate the NDIS, and use it to its full potential. Providers like Empowrd offer support to help you build capacity to maintain relationships, manage tasks, live independently, and engage more in the community.
Level 3: Specialist Support Coordination
Specialist Support Coordination is for NDIS participants with complex needs. A specialist support coordinator will help you manage the challenges in your support situation and ensure you receive reliable services.
